Total Testosterone vs. Free Testosterone vs. Bio Avail Testosterone

I recently posted some lab work that showed a high total testosterone level due to exogenous supplementation (4890ng/dL) . Included in the results was a reading on "bio avail" testosterone. This number was 4157 ng/dL, which seemed very high relative to total test. I mistakenly thought at first that bio avail test is equivalent to "free" test. Free test represents all the testosterone in your blood that is not bound to SHBG, and is thus available for use. Bio avail test is actually a more accurate reading of how much testosterone is available for use by your tissues, because there is a significant amount of testosterone that is bound very lightly by Albumin, which will not register on the lab work as free test, but is nonetheless easily taken up in the capillary beds of different tissues.

So, this explains why my bio avail test was almost as high as my total test. I hope this makes sense, and it certainly is nice to know I don't have low AR receptor density and/or low SHBG levels!

That makes a lot more sense. Wow, was I wrong there, Bioavailable =/= Free. (and it was so easily available too with a google search!). Good job looking into this, GS.

You are exactly right my man. I thought someone had said that bio and free test were the same so I took a look. Free testosterone measures the free fraction, bioavailable testosterone includes free plus weakly bound to albumin. Thank you for being such an incredible contributor here. Makes me want to keep coming back.

yes very good work my man ! The writing I had read had said that bio - available test was the same as free test , just goes to show you have to look into things more throughly. Being on this board is like going to school - GEAR school. lol

She's hot, and a great skier. But a total bitch IRL (apparently).

She had a bad crash in Feb. I'm amazed she can even bone Tiger. Maybe he just likes her company...
